If you're initially applying to a Blocking Certificate through COMO Digital Life:
Passport/ID card copies (the copies must not be older than three months)
If you're applying directly to a COMO Digital Life Service:
Extract from the commercial register (not older than three months)
Extract from the shareholder register (not older than three months)
Company's articles of association (the copy must originate from an official government website or be a certified true copy)
Passport/ID card copies (the copies must not be older than three months) for directors, UBOs and legal representatives.
Proof of address (not older than three months)
